Referrals from Mersey Care

We work in partnership with Mersey Care NHS Foundation Trust Community Mental Health Teams to support patients with their mental health and wellbeing. We work alongside the other organisations involved in each person's care, so their support is consistent and person-centred.

Which teams can refer

  • Speke CMHT

    Moss House

  • Kirkby CMHT

    Northwood House

Patients must currently be working with one of these teams, and the referral must come from a professional in the team.

Support we offer patients

  • Wellbeing support

    Accessible wellbeing sessions from our facilitators on managing emotions, relationships, trauma, stress and anxiety, to help improve overall health and wellbeing.

  • Bespoke support

    Depending on the person's needs, specialised group work and bespoke one-to-one sessions to help with specific mental health needs.

  • Step-up support

    Wrap-around support from our team: consistent one-to-one sessions, working closely with other health teams to make sure the person is fully supported.

How to refer

  1. Make the referral on RiO, the Mersey Care patient record system, as you would for other services.
  2. We contact you or the patient as soon as we can, usually within 1 week, to explain the support we have available.
  3. We agree the right support with the patient, depending on their needs, and keep working with the rest of their care team.

Frequently asked questions

Who can access the Mersey Care support?

This support is open to patients currently working with a Community Mental Health Team (CMHT) within Mersey Care NHS Foundation Trust. A professional in the Community Mental Health Team makes the referral through RiO.

Which teams can refer?

We currently accept referrals from the Speke CMHT (Moss House) and the Kirkby CMHT (Northwood House).

What happens when a referral is made?

Once the referral is made on RiO, the Mind Connect team will get in touch with the referrer or patient as soon as we can (usually within 1 week) to explain and offer the support we have available, depending on the person's needs.

What if I want support but am not under a Community Mental Health Team?

We offer free support and wellbeing sessions in Speke every Monday and Garston every Wednesday. You don't need a referral. See our Get Support page for details.
